Research on the current situation of exercise fear in COPD patients and the application of assessment scales
COPD患者运动恐惧现状及量表应用研究

摘要

目的 探讨慢性阻塞性肺疾病（COPD）患者运动恐惧现状，分析运动恐惧评估量表的临床应用价值，对比不同康复干预下COPD患者运动恐惧水平差异，为临床优化肺康复方案、缓解患者运动恐惧心理提供参考。方法 筛选2024年1月—2025年12月在本院诊疗的80例稳定期慢阻肺患者作为研究样本，随机数字表法分为两组，每组40例。对照组给予常规呼吸内科护理干预，观察组在对照组干预基础上，施以精准化肺康复矫治配合心理调适干预模式。比较两组干预后运动恐惧量表评分、量表相关性分析及干预后运动恐惧改善效。结果 经为期3个月干预，两组TSK、BBQ量表总得分与各维度分值均相较于干预前显著下降，观察组全部测评指标分值更低，组间对比具备统计学差异（P＜0.05）；经Pearson相关性分析显示，TSK与BBQ量表评分呈显著正相关（P＜0.05）。两量表信效度良好，可有效评估COPD患者运动恐惧程度。结论 COPD患者普遍存在明显的运动恐惧心理，Tampa运动恐惧量表与呼吸困难信念问卷可作为该人群运动恐惧的有效评估工具，针对性干预措施可显著缓解患者运动恐惧，提升康复效果。

Abstract

Objective To investigate the current status of kinesiophobia in patients with chronic obstructive pulmonary disease (COPD), analyze the clinical application value of kinesiophobia assessment scales, compare the differences in kinesiophobia levels among COPD patients receiving different rehabilitation interventions, and provide references for clinically optimizing pulmonary rehabilitation protocols and alleviating patients’ kinesiophobia.
Methods A total of 80 patients with stable COPD treated in our hospital from January 2024 to December 2025 were selected as research samples and divided into two groups via the random number table method, with 40 cases in each group. The control group received routine nursing intervention for respiratory medicine, while the observation group received targeted pulmonary rehabilitation correction combined with psychological regulation intervention on the basis of the control group. The post-intervention scores of kinesiophobia scales, correlation analysis of scales, and improvement efficacy of kinesiophobia after intervention were compared between the two groups.
Results After 3 months of intervention, the total scores and dimensional scores of the TSK and BBQ scales in both groups decreased markedly compared with baseline, and all indicator scores of the observation group were lower, with statistically significant inter-group differences (P＜0.05). Pearson correlation analysis demonstrated a significant positive correlation between TSK and BBQ scale scores (P＜0.05). Both scales possess good reliability and validity and can effectively evaluate the severity of kinesiophobia in COPD patients;
Conclusion   COPD patients generally have obvious exercise fear psychology. The Tampa exercise fear scale and the breathing difficulty belief questionnaire can be effective assessment tools for this population’s exercise fear, and targeted intervention measures can significantly alleviate patients’ exercise fear and improve rehabilitation effects.
