摘要
目的 讨论及研究医护患一体化慢病管理模式对维持性血透患者血红蛋白达标率影响的效果。方法参与本次研究患者的数量为100例，研究的起始时间为2025年1月份，截止时间为2025年12月份，分成对照组和实验组，每组50例，对照组常规护理，实验组为医护患一体化慢病管理模式，对比两组患者的血红蛋白水平、血红蛋白压达标率，评估患者护理管理满意度。结果 两组患者提供不同的护理方案后，实验组患者血红蛋白水平优于对照组，p<0.05，对比患者血红蛋白达标率，实验组高于对照组，p<0.05，对比患者护理满意度，实验组高于对照组，p<0.05。结论 对维持性血透析患者提供医护患一体化慢病管理，在改善患者血红蛋白水平的同时，提高了患者的血红蛋白达标率，提升了患者的护理满意度，值得提倡。

Abstract
Objective To discuss and study the effect of an integrated medical-nursing-patient chronic disease management model on the hemoglobin target achievement rate in maintenance hemodialysis patients. Methods A total of 100 patients participated in this study, which started in January 2025 and ended in December 2025. They were divided into a control group and an experimental group, with50 patients in each group. The control group received routine nursing care, while the experimental group received integrated medical-nursing-patient chronic disease management. The hemoglobin levels, hemoglobin target achievement rates, and patient satisfaction with nursing management were compared between the two groups. Results After providing different nursing plans, the hemoglobin levels in the experimental group were better than those in the control group (p<0.05). The hemoglobin target achievement rate was also higher in the experimental group than in the control group (p<0.05). Patient satisfaction with nursing care was also higher in the experimental group than in the control group (p<0.05). Conclusion Providing integrated medical, nursing, and patient-centered chronic disease management for maintenance hemodialysis patients not only improves hemoglobin levels but also increases the rate of achieving target hemoglobin levels and enhances patient satisfaction. This approach is worthy of promotion.
