摘要
目的 分析俯卧位通气干预护理对呼吸机辅助通气极早产儿的应用效果。方法 拟选取2024年4月至2025年8月呼吸机辅助通气极早产儿80例为研究对象，按随机数字法将其随机分为对照组（n=40，仰卧位通气治疗）和观察组（n=40，给予俯卧位通气治疗）。比较两组呼吸机通气时长、氧合指数、吸氧时间及动脉血气指标，比较两组并发症发生率。结果 观察组呼吸机通气时长、吸氧时间数值比对照组低，差异有统计学意义，P＜0.05；观察组氧合指数数值比对照组高，差异有统计学意义，P＜0.05。干预前两组患儿各项血气指标均较差，且无统计学意义，P＞0.05，干预后两组患者血气指标都有所改善，且观察组患者各项血气指标均明显优于对照组，差异有统计学意义，P＜0.05。观察组并发症发生率低于对照组，指标对比差异有统计学意义，P＜0.05。结论 俯卧位通气干预护理对呼吸机辅助通气极早产儿具有显著应用价值，可改善其血气指标、氧合指数，缩短通气及吸氧时间，降低并发症的发生。

Abstract
Objective To analyze the effect of prone position nursing intervention on extremely premature infants receiving ventilator-assisted ventilation. Methods Eighty extremely premature infants receiving ventilator-assisted ventilation from April 2024 to August 2025 were randomly divided into a control group (n=40, receiving supine position ventilation) and an observation group (n=40, receiving prone position ventilation). The two groups were compared in terms of duration of ventilator ventilation, oxygenation index, oxygenation time, and arterial blood gas parameters, as well as the incidence of complications. Results The duration of ventilator ventilation and oxygenation time in the observation group were significantly lower than those in the control group (P<0.05); the oxygenation index in the observation group was significantly higher than that in the control group (P<0.05). Before the intervention, all blood gas indices in both groups were poor and not statistically significant (P>0.05). After the intervention, blood gas indices improved in both groups, with all blood gas indices in the observation group significantly better than those in the control group (P<0.05). The complication rate in the observation group was lower than that in the control group, with statistically significant differences (P<0.05). Conclusion Prone positioning intervention nursing has significant application value for extremely premature infants receiving ventilator-assisted ventilation, improving blood gas indices and oxygenation index, shortening ventilation and oxygenation time, and reducing the incidence of complications.
