Application effect of continuity of care on glycaemic self-management in elderly patientswith type 2 diabetes mellitus
延续性护理对老年2型糖尿病患者血糖自我管理的应用效果

摘要

目的 分析老年2型糖尿病患者接受延续护理后的血糖自我管理效果。方法 在本院接受治疗的2型糖尿病患者中选择部分患者作为分析目标，共选择100例，于2022年12月-2023年12月完成选取，对相关资料进行统筹和整理，对所有患者进行排序后进行小组的划分，其中一个小组应用常规护理方式为参照组纳入50例患者，另一个小组采用延续护理视为研究纳入50例患者。统计相关情况进行分析。结果 统计患者血糖指标、自我管理依从性、满意度后，研究组优于参照组（P＜0.05）。结论 针对老年2型糖尿病患者采用延续性护理能够提高患者血糖自我管理能力，对血糖的改善有着积极作用，还能够提高满意度。

Abstract

Objective To analyse the effect of glycemic self-management in elderly patients with type 2 diabetes mellitus after receiving continuity care.
Methods Select some patients as the target of analysis among the patients with type 2 diabetes receiving treatment in our hospital, a total of 100 cases were selected, and the selection was completed from December 2022 to December 2023, the relevant information was co-ordinated and sorted out, and the division of the group was carried out after the sorting of all the patients, in which one group was included as the reference group by applying the conventional nursing care for 50 patients, and the other group was included as the study by applying the continuity of nursing care for 50 patients. 50 patients were considered to be included in the study. Statistical related situations were analysed.
Results After counting the patients' glycaemic indexes, self-management compliance and satisfaction, the study group was better than the reference group (P＜0.05).
Conclusion   The use of continuity of care for elderly patients with type 2 diabetes mellitus can improve the patients' ability to self-manage their blood glucose, which has a positive effect on the improvement of blood glucose, and also improves satisfaction.
