Effect of free position midwifery care on pregnancy outcome of primipara in natural delivery
自由体位助产护理对自然分娩初产妇妊娠结局的影响

摘要

目的 探讨对自然分娩初产妇实施自由体位助产护理的作用以及对妊娠结局的影响。方法 选取于我院产科中生产的100例初产妇作为研究对象，正式开展研究前已经征得了100例产妇的同意，同时其基本情况符合实验参与条件。为达成研究目的，对100例产妇随机分组，其中50例纳入至观察组，实施自由体位助产护理，另外50例则纳入至对照组，实施常规护理，比较两组产妇的产程时间、妊娠结局以及产痛和满意度评分。结果 据本次研究数据显示，观察组产妇第一产程时间（255.4±52.1）min，第二产程时间（35.2±7.7）min，对照组分别为（363.6±57.5）min、（47.8±6.2）min，观察组的产程时间更短（P＜0.05）；在妊娠结局方面，观察组产妇中阴道分娩率、剖宫产率分别为96.0%、4.0%，明显优于对照组82.0%、18.0%（P＜0.05）。最后，观察组患者的产痛评分和满意度评分也相对更优于对照组（P＜0.05）。结论 将自由体位助产护理实施于初产妇自然分娩中可显著缩短产程时间，有助于提高自然分娩率、减轻产痛，并提高产妇满意度。

Abstract

Objective To explore the effect of free position midwifery care for primipara in natural delivery and its influence on pregnancy outcome.
Methods 100 primiparas who gave birth in the obstetrics department of our hospital were selected as research objects. Consent of 100 primiparas was obtained before the formal study, and their basic conditions met the conditions for participation in the experiment. To achieve the purpose of the study, 100 parturients were randomly grouped, of which 50 were included in the observation group and received free position midwifery care, and the other 50 were included in the control group and received routine care. The duration of labor, pregnancy outcome, labor pain and satisfaction scores of the two groups were compared.
Results According to the data of this study, the first stage of labor time in the observation group was (255.4±52.1) min, and the second stage of labor time was (35.2±7.7) min while the control group was (363.6±57.5) min, (47.8±6.2) min, so the observation group was shorter (P < 0.05). In terms of pregnancy outcome, the vaginal delivery rate and cesarean section rate in the observation group were 96.0% and 4.0%, respectively, which were significantly better than those in the control group (82.0% and 18.0%, P < 0.05). Finally, the labor pain score and satisfaction score of the observation group were better than those of the control group (P < 0.05).
Conclusion   The implementation of free position midwifery care in the natural delivery of primipara can significantly shorten the labor time, help to improve the rate of natural delivery, reduce labor pain, and improve maternal satisfaction.
